Patient’s Terms of Service

By using Canopy to complete your Patient Registration, you agree to accept our Terms of Service and Privacy Policy.

  1. Oak Street Health takes health information privacy matters very seriously. We adhere strictly to HIPAA regulations (Health Insurance Portability and Accountability Act of 1996), which provide data privacy and security guidelines for safeguarding your medical information. Please see our Privacy Policy for more information.
  2. Only Oak Street Health employees have access to Canopy and will use it to complete the patient registration process.

Access and Use of Service 

Canopy Application: This service is designed to enable Oak Street Health employees to improve care delivered to individuals beginning, receiving or having received medical treatment, attention or care (“Patients”) from Oak Street Health. 

Canopy is an internal application and only Oak Street Health employees have user accounts. Oak Street Health patients may be asked to sign patient forms or provide information in Canopy during the Patient Registration process.

You Authorize Us to Use Your Data in Accordance With Our Privacy Policy: You hereby authorize the collection, storage, integration, transmission, sharing and/​or display by Canopy of personal data from health records or other personal data sources. 

Our privacy policy explains how we treat your personal information and protect your privacy when you use our Services. 

Compliance with Laws; Transmission of Data: With respect to its operation of the Services, and to the extent required by (i) the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the regulations promulgated thereunder, and (ii) the Health Information Technology for Economic and Clinical Health Act (“HITECH”) and any regulations promulgated thereunder (HIPAA and HITECH are collectively referred to herein as the Privacy Laws”), Canopy will fully comply with the Privacy Laws applicable to business associates and maintain the confidentiality of any Protected Health Information (as defined by the Privacy Laws) (“PHI”) transmitted or made available through the functionality of the Services or the Affiliated Services.You understand and agree that this Terms of Use Agreement may be amended from time to time if necessary to comply with the Privacy Laws. The requirements of this Section will survive the termination of Your use of the Services.

To the extent permissible under the Privacy Laws and any other applicable Federal and State laws, You hereby grant to Canopy a perpetual, unlimited license to use the data and information that is compiled or passes through the Services that specifically relates to You, Your patient care, Your Provider’s procedures or diagnoses, or insurance provider procedures, policies, coverage, or payments, and any related information (collectively, the Client Data”), in a de-identified format as defined under the Privacy Laws for data benchmarking, sharing, warehousing, resource utilization, and similar data analysis services; provided, however, that Canopy shall comply with the Privacy Laws in connection with any such actions.

About these Terms

We may modify these terms or any additional terms that apply to Canopy to, for example, reflect changes to the law or changes to Canopy. You should look at the terms regularly and the Last Updated” date at the beginning of these terms. We’ll post notice of modifications to these terms on this page. Changes will not apply retroactively and will become effective no sooner than ten (10) days after they are posted. However, changes addressing new functions in Canopy or changes made for legal reasons will be effective immediately.

These terms control the relationship between your information in the Canopy application and you. They do not create any third party beneficiary rights. If you do not comply with these terms, and we don’t take action right away, this doesn’t mean that we are giving up any rights that we may have (such as taking action in the future). If it turns out that a particular term is not enforceable, this will not affect any other terms.

You may not assign or delegate your rights or obligations relating to these terms or your account for the Services without our prior written consent. We may assign these terms or assign or delegate any of our rights or obligations at any time.
