CHW Cares Providers Affiliate with Oak Street Health

Many of the CHW Cares Providers at 4405 Broadway have agreed to affiliate with Oak Street Health. At the end of September, Oak Street Health took over the location and is in custody of your medical records. These CHW Cares Providers will continue to provide care at the same location — 4405 Broadway.

Oak Street Health is committed to your long-term health goals. Oak Street Health will offer additional services such as a 24⁄7 live support line for around-the-clock support and extra staff dedicated to helping you get the most from your Medicare benefits. While Oak Street Health has historically focused on primary care, it will continue to offer a handful of specialty services at this location.

These Providers and Oak Street Health are pleased to continue your medical treatment.
