    To protect yourself and your loved ones from COVID-19, TAKE 5”:

    1. Stay at home.
    2. Wash your hands
    3. Stick to your care plan. Call Oak Street Health at 844-871‑5651 if you’re in need of a primary care provider
    4. Call Oak Street Health if you’re feeling sick or need help.
    5. Do not visit the Emergency Room unless you’re experiencing a medical emergency.

    Social distancing is a way to help stop the spread of COVID-19, which is very contagious. We are encouraging our patients to stay home as much as possible. We understand you may need to go to work, visit a store for essential items or make a necessary medical visit. When leaving the house for these reasons, keep about 6 ft of space between you and others, avoid people who are sick, and wash your hands often.
