Oak Street Health and UnitedHealthcare Establish New Network Relationship

People enrolled in UnitedHealthcare Medicare Advantage plans will have access to Oak Street Health’s innovative value-based approach to patient care, having resources committed to sharing information and identifying care opportunities.

CHICAGO (Nov. 30, 2017) – Oak Street Health and UnitedHealthcare have established a new network relationship, providing Chicagoans enrolled in UnitedHealthcare Medicare Advantage health plans access to Oak Street Health’s 14 primary care centers in Illinois, effective Nov. 1, 2017, and the health system’s two locations in northwestern Indiana beginning Dec. 1, 2017.

Oak Street Health’s value-based care model promotes a team approach to health care that improves quality, lowers cost and enhances care coordination. This relationship will provide access to the 20,000 people enrolled in UnitedHealthcare Medicare Advantage plans in the greater Chicago area.

Key to the success of this new relationship is UnitedHealthcare sharing data with Oak Street Health to inform doctors of patients’ underlying medical conditions, past treatments, gaps in care, medications prescribed and future care needs. This takes the burden off patients from having to connect information from each of their doctor’s visits, reduces duplicative tests, and improves care coordination across specialties and care centers.

Through this initiative, UnitedHealthcare and Oak Street are able to identify clear, actionable information specific to individual patients’ health needs. This collaboration will help improve the ability to identify patients who are at high risk and help them reduce emergency room visits and re-admissions to the hospital, manage their chronic health conditions and more efficiently manage their medication needs.

Care providers nationwide are showing strong interest in a shift to value-based care. UnitedHealthcare’s total payments to physicians and hospitals that are tied to value-based arrangements have tripled in the last three years to more than $54 billion. By the end of 2018, UnitedHealthcare expects that figure to reach $65 billion.
