Dear Oak Street Health Patient,

Our community is facing a challenge from the Coronavirus/COVID-19 pandemic. Oak Street Health is here for you. As your primary care provider, our goal is to keep you happy, healthy and out of the hospital.

COVID-19 presents a high risk to seniors. Government leaders and health officials are instructing seniors to stay at home.

Our new telehealth appointments will provide you with high-quality primary care without leaving your home. These are phone or video visits. It is important to continue to attend these visits so that you can stay healthy.

Oak Street Health remains open for in-person visits, as deemed medically-necessary by your doctor.

What about COVID-19 testing? If you are sick with COVID-19 symptoms (fever, cough, shortness of breath), your Oak Street Health provider can assess you via telehealth and determine if you need a test.

We know that staying at home can be hard. If you need food, medicine, or anything else, please call us and we will do our best to help.

  2. Wash your hands.
  3. Stick to your current care plan.
  4. Call Oak Street Health at (844–871-5655) if you’re feeling sick or need help.
  5. Do not visit the Emergency Room unless you’re experiencing a medical emergency.
