Nurse Practitioner Fellowship — September 2024 Cohort (Heartland Division)

- Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2022 and May 2024.
- Currently residing in or willing to relocate to program location for at least the next two years. Relocation assistance is available.
- Have not yet worked as an NP.
- Minimum of one year experience working as an adult RN.
- Minimum GPA of 3.0 in NP program.
- Currently holds or will hold by 3/1/2024 an active, non-probationary state NP license.
- A genuine passion for adult primary care.
- A supportive attitude toward our patient population of older adults.
- Embrace teamwork and the opportunity to collaborate with brilliant colleagues.
- Want to be a part of an innovative model focused on empirically guided population health.
- Spanish language skills are strongly preferred, depending on clinic location.
