What to expect from your first appointment.
At your first appointment we’ll discuss your healthcare goals and what we can accomplish in the next year together:
- Meet with your primary care provider for an initial exam to discuss your history and plan out your health journey.
- Meet with your full care team of primary care and osteopathic doctors, nurse practitioners, and physician assistants and understand how they support you on this journey.
- Get answers to your Medicare insurance questions.
Prepare ahead of time
Please arrive early for your appointment so you have time to fill out any paperwork. Or, for your convenience, you can print out the documents at home and have them ready when you arrive.

Remember to bring to your appointment:
- Two forms of ID
- Your Medicare Insurance card
- Any medication you are currently taking, both over-the-counter and prescription drugs
