NEW YORK — December 4, 2023 — Oak Street Health, a network of value-based primary care centers for adults on Medicare and the only primary care provider to carry the AARP name, is now welcoming patients at the new Hempstead center at 210 Fulton Ave in Hempstead. This is the 18th center for Oak Street Health in the state of New York and follows the opening of the Freeport center in New York that opened in July.
