My visits have been very good with oak Street and Dr.Upton

She was quick, I gave her a form for diabetic shoes from my foot doctor, told her everything was filled out,all she had to do was sign and fax. They said they never received it. Almost 3mon. Since I've seen her. I've had major surgery since, and going to rehab, still haven't heard from her.
