Below is a brief look at what you can expect as a patient with us. We’ll take the time to listen, understand your unique needs, and make sure you have the support you need to stay healthy.
Your New Patient Visit
This visit is the first step to becoming a patient. It’s a chance for us to get to know each other, and for you to learn about how we care for patients.
What to expect
During this visit, you’ll meet your new primary care provider and care team. We’ll explain how care works here, and you can ask questions. A team member will also go over your insurance benefits and answer your Medicare questions.
What will happen
We’ll check your vitals, review your medications, and talk about your health history and goals. If you choose to become a patient, we’ll guide you through the process to update your primary care provider on your insurance.
What to bring
Please bring your insurance cards, photo ID, list of past and current medications, emergency contact information, and list of past and current doctors.

Your Health Review Visit
This is your second appointment with Oak Street Health. It’s an opportunity for us to check in and make sure you’re getting the care you need.
What to expect
Your health review visit is an important checkup. During this visit, your provider will take a complete look at your health. You’ll have time to share questions or concerns as well.
What will happen
You’ll review your medications with your provider, and discuss any long-term conditions you may have. Depending on your needs, you may receive a physical exam. We’ll also talk about any vaccines or screenings you may need. Together, we’ll decide on next steps.
What to bring
Please bring your insurance cards, photo ID, and list of your current medications. It may be helpful to write down any questions or new symptoms before your visit.

Medicare Wellness Visit
Next, we’ll schedule your Medicare Wellness Visit. We’ll talk about your lifestyle and family history, and do a few simple tests to check your memory and mental health. Then together, we’ll make a plan to help you stay ahead of health issues.
Ongoing care
We want to check in on you regularly to help you stay healthy. Based on your needs, your provider will decide how often we should see you. And, if you ever feel sick or have questions before your next visit, just call us. We’re here to help.
