Oak Street Health Fuels National Expansion, Enters Tennessee and Texas in 2020

Oak Street Health, a network of primary care centers that delivers value-based care to adults on Medicare, today announced plans for significant nationwide expansion in 2020, continuing its impressive growth trajectory to date. The company will mark its entry into Tennessee and Texas with centers in Memphis and Dallas-Fort Worth. Oak Street Health will also increase its presence in existing markets, with additional centers opening in Detroit, Cleveland, and the Greensboro-High Point metro area in North Carolina. This growth will significantly increase access to Oak Street Health’s innovative approach to primary care for older adults across the country.

Patients who visit an Oak Street Health center will have a healthcare experience they may not have encountered before, including extended time with their provider and individualized treatment plans, community-centered support for social wellness, a 24/7 patient support line, and access to transportation to and from the center for eligible patients. Supplementary services, such as behavioral health support and Medicare education classes, are also offered to help older adults meet their unique needs in one convenient location. Since its founding in 2012, Oak Street Health has seen a 41 percent reduction in patient hospital visits compared to Medicare benchmarks.

Oak Street Health will open two locations in Memphis within the Frayser and Audubon Park neighborhoods this winter, followed by Dallas-Fort Worth’s first center this summer. New openings across Cleveland, Detroit and the Greensboro-High Point metro area, North Carolina are also expected to open in late spring. Oak Street Health will accept multiple health plans at all locations, including traditional Medicare.
