Oak Street Health Opening Three New Centers in Michigan

Value-based Primary Care Set to Open Centers in Roseville, Pontiac and Highland Park in August

DETROIT, MI — August 10, 2020 — Oak Street Health, a fast-growing network of value-based, primary care centers for adults on Medicare, is ready to begin caring for more seniors in the greater Detroit area with the opening of three new centers in Roseville, Pontiac and Highland Park. 

• The Eastgate center located at 26390 Gratiot Avenue in Roseville is projected to open on August 10.
• The Pontiac center located at 462 N. Telegraph Road in Pontiac is projected to open August 17.
• The Woodward center located at 14507 Woodward Avenue in Highland Park is projected to open August 24.

Since its founding in 2012, Oak Street Health has seen a 51 percent reduction in patient hospital admissions compared to Medicare benchmarks. Oak Street Health will accept multiple health plans at all locations, including traditional Medicare.
