Oak Street Health Opens New Center in Cleveland

CLEVELAND, OH — August 24, 2020 — Oak Street Health, a fast-growing network of value-based, primary care centers for adults on Medicare, is ready to begin caring for more seniors in Cleveland with the opening of its newest center at 6410 Broadway Avenue on August 24. The opening brings the number of Oak Street Health centers in Cleveland to four and the number of centers in Ohio to seven, joining existing centers in Dayton and Youngstown.
